Appointment Deposits
To secure your appointment at SM Aesthetic Lab, a deposit of $50.00 per treatment selected is required. This deposit is redeemable towards your treatment or refundable if unused. However, in the event of a 'No-Show,' where the client fails to attend their appointment without prior 48-hour notice, the deposit will be forfeited and recorded for future reference.
Cancellation and Amendment Policy
We kindly request that any cancellations or amendments to your appointment be made at least 48 hours in advance. Failure to provide sufficient notice may result in the forfeiture of your deposit. This policy ensures that we can manage our schedule effectively and offer appointments to other clients in need.
Client Information and Privacy
To ensure the safe and effective delivery of treatments, SM Aesthetic Lab requires the collection of private information. For Skin & Laser treatments, new clients must complete a Client Intake Form, a New Medical Form (to assess the suitability and safety of the treatment), and a Treatment Consent Form. For cosmetic procedures, a Client Intake Form and a New Medical Form are legally required to ensure accurate scripting and safety. Additionally, a treatment-specific consent form must be completed before any procedure.
All personal information collected is treated as strictly confidential and will not be shared outside of SM Aesthetic Lab unless legally necessary. It is essential to disclose any relevant health information to your Doctor, Nurse, or Dermal Therapist to ensure your safety and the appropriate treatment is administered.
Photography and Consent
As part of your treatment, photos may be taken before, during, and after the procedure. These images are securely stored on devices owned by SM Aesthetic Lab. If we intend to use your photos for any advertisement or educational purposes, you will be asked to complete a Photo Consent Form. If you choose not to complete this form, your photos will be stored exclusively for medical records and will not be used externally.
Privacy and Data Protection
SM Aesthetic Lab is committed to protecting your personal information in accordance with the Australian Privacy Principles (APPs) as outlined in the Privacy Act 1988 (Cth). We collect, use, disclose, store, and dispose of your information in compliance with these principles.
Sensitive information, as defined by the Privacy Act, will only be used for the primary purpose it was collected, or for a secondary purpose directly related to the primary purpose, with your consent, or as required by law.
Disclosure of Personal Information
Your personal information may be disclosed to third parties with your consent or as required by law. We ensure that your information is stored securely and protected from unauthorised access, modification, or disclosure.
Security and Retention of Personal Information
SM Aesthetic Lab takes all reasonable measures to protect your personal information from misuse, loss, and unauthorised access. When your information is no longer required, it will be securely destroyed or permanently de-identified. However, client files are retained for a minimum of 7 years as per legal requirements.
Access to Your Personal Information
You have the right to access and update your personal information held by SM Aesthetic Lab. To request access, please contact us in writing. We may require identification to verify your request, and while there is no fee for access, an administrative fee may apply for copies of your information.
Maintaining Accurate Information
We strive to maintain accurate and up-to-date personal information. If your information changes or is inaccurate, please notify us promptly so we can update our records and continue providing quality services.
Returns and Exchanges for Product Purchases
SM Aesthetic Lab does not offer refunds on product purchases unless medically necessary. If you experience an allergic reaction to a product, a medical certificate is required to process a refund. Exchanges are permitted on unopened products within 30 days of purchase, due to hygiene and sanitary reasons.
